In the era of increased awareness of structural racism and its impact on health, this presentation will discuss the historical basis for health disparities, the impact of social determinants of health and the convergence of health disparities and social determinants of health and how they impact health and health outcomes.
At the conclusion of this presentation, participants will be able to:
- Discuss historical basis for health disparities and sources of health disparities
- Identify and describe social determinant of health and articulate the impact of social determinants of health on chronic disease outcomes
- Analyze the intersection of social determinant of health with the health care system and discuss potential solutions
